Abstract

Detection of neuraminidase inhibitors-resistant influenza virus through genetic and phenotypic analysis in Korea

The applicable anti-influenza drugs were classified into neuraminidase inhibitors (Oseltamivir, Zanamivir, Peramivir, and Lanaminivir), ion channel blockers (Rimantadine and Amantadine) and a cap-dependent endonuclease (Baloxzvir), according to their respective mechanisms. In South Korea, Oseltamivir, Zanamivir, and Peramivir are mainly prescribed for patients exhibiting influenza-like illness and reports of antiviral drug-resistant A(H1N1)pdm09 viruses are rare. The Korea Centers for Diseases Control and Prevention operates the Korea Influenza and Respiratory Viruses Surveillance System (KINRESS) to monitor epidemic features of influenza viruses and analyze viral characteristics, such as anti-drug resistance. First, the neuraminidase (NA) gene of type A or B influenza was analyzed to screen for resistance to neuraminidase inhibitors (NAIs). H275Y was found to be a major mutation that inhibited NAIs application, especially Oseltamivir, against A(H1N1)pdm09 infection. After confirming the presence of mutations in NA function, NA activity of isolated viruses was calculated via a fluorescence-based assay using 4-(methylumbelliferyl)-N-acetylneuraminic acid (MUNANA) substrate. Neuramindase inhibition assay was conducted by calculating NA activity of NAIs-treated viruses. KINRESS aims to monitor patients with influenza-like illness who visit clinics (primary hospitals). NAIs-resistant virus was not detected during KINRESS surveillance for the past five seasons. However, recently A(H1N1)pdm09 viruses harboring the H275Y mutation in NA were detected in two hematological malignancy patients infected with influenza. The patients exhibited prolonged respiratory symptoms, with released viruses in the respiratory tract in spite of Oseltamivir and Peramivir application. The results of neuraminidase inhibition assay indicated that the viruses were resistant to Oseltamivir and Peramivir, but not Zanamivir. Therefore, it is essential to monitor influenza-like illness in both inpatients and outpatients, including those who are immunocompromised.

Neuraminidase inhibitors(NAIs)는 인플루엔자 A형과 B형의 치료 및 예방 목적으로 전 세계적으로 사용되는 약물이며[1] 국가별로 신종인플루엔자 대유행을 대비하여 Oseltamivir와 같은 인플루엔자 치료제를 비축하고 있다. NAIs는 인플루엔자 바이러스 표면에 있는 neuraminidase 효소의 작용을 억제함으로써 감염된 세포로부터 증식된 바이러스(progeny virus)가 세포 밖으로 퍼져나가는 것을 막는다[2]. 인플루엔자 치료제로 가장 많이 사용되는 Oseltamivir는 경구제제로 인플루엔자 증상 발현 후 2일 이내 복용을 권장하며 임상증상을 경감시키고 호흡기 기관 내 바이러스의 배출 기간을 단축시킨다[1]. 이외에도 cap-dependent endonuclease inhibitor로 알려진 Baloxavir[3]는 최근 일본에 이어 미국에서도 인플루엔자 치료제 사용허가를 받았으며, Laninamivir는 1회 투여 호흡기 치료제로 일본에서만 적용하고 있다[4].
최근 검출되는 대부분의 A형 인플루엔자 바이러스는 Matrix 유전자에 내성변이(S31N)를 지니므로 M2 ion channel blocker로 알려진 Rimantadine과 Amantadine은 더 이상 임상에 적용되지 않고 있다[5]. 국내에서 주로 사용되는 NAIs는 Oseltamivir, Zanamivir, Peramivir가 있으며 각각 경구, 호흡기, 정맥으로 투여한다.
세계보건기구에서는 세계 인플루엔자 감시 및 대응 체계(Global Influenza Surveillance and Response System, GISRS)를 운영하여 144개국의 국립인플루엔자센터(National Influenza Center, NIC)로부터 검체 및 바이러스 공유를 통하여 치료제 내성 분석을 수행하고 있으며 매 절기 NAIs에 대한 분석 결과를 보고하고 있다[6]. 질병관리본부 바이러스분석과는 국립인플루엔자센터로서 인플루엔자 및 호흡기바이러스 병원체 감시사업 운영과 민·관원 진단검사를 수행하고 있으며 인플루엔자 바이러스의 유전형 분석 및 표현형 분석을 통하여 NAIs 치료제에 대한 내성을 지니는 인플루엔자 바이러스를 지속적으로 모니터링하고 있다.
인플루엔자 치료제 내성을 확인하기 위하여 neuraminidase(NA) 유전자 중 치료제 내성 변이가 확인되면 바이러스를 분리한 후 표현형 분석(Neuraminidase inhibition assay)을 수행한다. NAIs 치료제 내성주 A(H1N1)pdm09 및 A(H3N2)에 대한 대표적인 NA 유전자 변이는 각각 H275Y 및 E119V 이며[7], 그 외에도 다양한 유전자 변이들이 NAIs 치료제 내성과 관련이 있다[8]. NA 유전자에서 치료제 내성이 확인된 바이러스는 세포 접종을 통하여 분리하여 치료제 표현형 분석법인 neuraminidase inhibition assay를 수행한다. 분리된 바이러스는 NA activity를 측정하여 바이러스 희석배수를 산출한 후 NAIs 치료제로 처리하여 희석된 바이러스의 NA activity를 측정한다. 분석하고자 하는 내성의심 바이러스의 IC50 값을 NAIs 치료제 감수성주의 IC50 값으로 나누어 fold change를 산출하여 내성의 정도를 평가한다[9]. NAIs 치료제 내성주는 1, 2차 병원의 외래환자보다는 주로 3차 병원에 입원한 면역저하 환자와 같은 인플루엔자 고위험군에서 NAIs 치료제 적용 후에도 지속적으로 바이러스가 검출되는 경우 나타난다고 알려져 있다[10]. 이 글에서는 최근 민원으로 의뢰된 혈액암 환자로부터 검출된 A(H1N1)pdm09 바이러스의 치료제 내성 분석 결과를 보고하고자 한다.

1. NAIs 치료제 내성 스크리닝을 위한 유전자 분석

인플루엔자 바이러스의 NA 유전자 분석은 인플루엔자가 확인된 검체에서 가장 빠르게 NAIs 치료제 내성을 확인할 수 있는 방법이다. NAIs 치료제 내성과 관련된 변이인 H275Y는 Oseltamivir와 Peramivir 내성을 갖는 A(H1N1)pdm09에서 나타나는 주된 변이이며 이외에도 E119D/G, Q136R 등의 변이가 있다. A(H3N2)와 관련된 NAIs 치료제 내성 변이로는 E119V, D151G/V/D, R224K, E276D, R292K 및 N294S 등이 있으며 B형 인플루엔자 바이러스의 경우 G104E, E117A/D, H134Y 및 R150K 등이 있으며 관련 변이들이 복합적으로 나타나서 더 높은 치료제 내성을 보이기도 한다. 이와 같은 유전자 변이에 의한 NAIs 내성 여부는 다양한 연구를 통하여 증명되었다[8](Table 1).

질병관리본부 감염병분석센터 바이러스분석과에서 운영하는 인플루엔자 및 호흡기바이러스 병원체 감시(Korea Influenza and Respiratory Viruses Surveillance System, KINRESS)를 통하여 최근 5절기 동안 1차병원에 내원하는 인플루엔자 의심(Influenza-like illness) 환자를 대상으로 검출된 인플루엔자 바이러스의 NA 유전자 분석결과 NAIs 치료제 내성과 관련된 변이를 확인할 수 없었다(Table 2).

하지만 최근 민원으로 의뢰된 2건의 면역저하 환자 검체에서 A(H1N1)pdm09이 검출되었으며 Oseltamivir와 Peramivir 투여 후에도 임상증상에 호전이 없고 지속적으로 인플루엔자가 검출된다는 담당의사의 소견에 따라 NA 유전자 검사를 수행한 결과 2건 모두 NA 유전자에서 H275Y 변이를 확인하였다.

2. 표현형 분석을 위한 phenotyping assay(Neuraminidase inhibition assay)

Neuraminidase inhibition assay 방법으로는 fluorescence-based(FL) assay와 chemiluminescence-based(CL) assay가 있으나 FL assay가 경제적이고 효율적이므로 치료제 내성 표현형 분석법으로 주로 사용된다[11]. 반응에 사용할 바이러스의 희석 농도를 결정하기 위하여 (Normalization) 바이러스를 2배씩 희석(2 fold dilution)하고 4-(methylumbelliferyl)-N-acetylneuraminic acid(MUNANA) solution에 반응시켜 florescence reader(wavelengths 355/460)로 NA activity를 측정한다. 각각의 바이러스는 NA activity 28,000 fluorescence unit에 맞도록 희석하였다. 희석된 바이러스에 5배씩 희석한(5 fold dilution, 0∼30,000 nM) 바이러스 억제제(Oseltamivir, Peramivir, Zanamivir)를 반응시킨 후 NA activity를 측정하여 IC50(NA activity를 절반으로 낮추는데 필요한 치료제 농도) 값을 산출한다. 최종적으로 NAIs 치료제 내성을 판별하기 위하여 분석하고자 하는 바이러스의 IC50 값을 NAIs 치료제 감수성 바이러스의 IC50 값으로 나누어 fold change를 산출하며 fold change에 따라 normal inhibition(NI), reduced inhibition(RI), highly reduced inhibition(HRI)으로 판정한다[12](Table 3).

A(H1N1)pdm09, A(H3N2) 및 B형에 대한 NAIs 치료제 내성 표준주를 neuraminidase inhibition assay로 분석한 결과 NA 유전자에 주요 변이를 갖는 내성주에서 highly reduced inhibition 또는 reduced inhibition을 확인할 수 있었다(Table 4).

민원으로 의뢰된 면역저하 환자에서 A(H1N1)pdm09 바이러스를 분리하여 neuraminidase inhibition assay를 수행한 결과 Oseltamivir와 Peramivir에 대한 강한 내성과 Zanamivir에 대하여 감수성 결과를 확인하였다(Table 5).

맺는 말

NA 유전자의 H275Y 변이를 지니는 NAIs 치료제 내성 A(H1N1)은 2007년 처음 확인되었으며 전 세계적으로 2008-2009절기에 유행하였다[14]. 하지만 2009년 신종인플루엔자 발생 이후 A(H1N1)pdm09에서는 약 1% 이하로 발견되고 있으며 주로 암, 장기이식과 같은 면역저하 환자에서 보고되고 있다[15]. 면역저하 환자가 인플루엔자에 감염되면 Oseltamivir 투여 후에도 지속적인 바이러스가 검출되며 감염 후 일주일 이후에는 H275Y 변이가 생길 수 있다는 것을 면역저하 족제비 모델을 통하여 증명된 바 있다[16].
본 사례에서도 면역이 저하된 혈액암 환자로부터 검출된 A(H1N1)pdm09의 NA 유전자에서 H275Y 변이가 확인되었으며 neuraminidase inhibition assay 결과에서도 Oseltamivir와 Peramivir에 대한 내성이 확인되었다. NAIs 치료제 적용 전·후의 호흡기 검체를 확보하여 치료제 내성 비교 분석을 수행하지는 못하였지만 환자의 호흡기도 내에서 지속적으로 바이러스가 검출된 것으로 보아 본 내성주의 출현은 지역사회감염 보다는 면역저하 환자에서 나타나는 사례로 의심된다. 두 사례에서 확인된 바이러스의 HA 유전자를 분석한 결과, 주요한 아미노산 변이 3개가 확인되어 동일한 바이러스에 의한 감염이 아님을 확인할 수 있었다.
본 연구를 통하여 면역저하 환자에서 A(H1N1)pdm09 감염으로 Oseltamivir와 Peramivir를 적용함에도 불구하고 지속적으로 바이러스가 검출되는 경우 Zanamivir 투여를 고려할 수 있으며, 내성 바이러스의 감염경로를 확인하기 위하여 NAIs 치료제 적용 전·후 호흡기 검체 확보의 필요성과 중증급성호흡기감염(Severe Acute Respiratory Infection, SARI) 감시체계 강화를 통하여 면역저하 환자와 같은 인플루엔자 고위험군에서의 NAIs 치료제 내성주 모니터링의 필요성을 제시하였다.
